I, the undersigned parent or legal guardian, consent to my child’s participation in the Little Yogi class offered by Reflections Yoga and Wellness LLC. I understand that yoga involves physical activity that may include stretching, balance, and movement exercises. I acknowledge that, as with any physical activity, there is a risk of injury. I affirm that my child is in good health and physically capable of participating in this program.

Release of Liability:

In consideration of my child’s participation in the Little Yogi class, I hereby release and hold harmless Reflections Yoga and Wellness LLC, its owners, instructors, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury, that may be sustained by my child during participation in this program.

Media Release:

I grant permission for my child to be photographed or videotaped during the Little Yogi class. I understand that these images may be used for promotional purposes, including but not limited to social media, newsletters, and the organization's website.

I do NOT grant permission for my child to be photographed or videotaped.

Acknowledgment and Signature:

I have read and fully understand the terms of this waiver and release. I acknowledge that I am voluntarily signing this agreement.
